Who Qualifies?

A patient may qualify for home care if he or she has experienced one or more of the following:

• Hospital or skilled nursing facility discharge
• Home care is requested by your physician during an office visit
• Ongoing health issue(s)
• Difficulty caring for yourself due to chronic illness
• A concerned family member
• Need help or education to manage your medication, health condition, or pain and symptom
• Change in mobility or a decline in the ability to complete activities of daily living
• Decline in mental status
• You may qualify for hospice or benefit from palliative support if you have been diagnosed with a terminal disease
