At Genesys, Reverence Home Health & Hospice provides hospice care services – both in the home and residential – through an exclusive partnership with Genesys Hospice. Our range of services includes comprehensive, home-based hospice care and support for terminally ill patients – 24 hours a day, seven days a week – to full-service residential care at the Genesys Hospice Care Center. We involve the patient and their loved ones in the care planning process to provide personalized services based on the patient’s individual needs and symptoms with a special emphasis on managing a patient’s pain and discomfort.
The Genesys Hospice team of caring specialists focuses on providing support, quality care and helping patients maintain dignity, while they live their remaining time as fully as possible. Physicians, nurses, medical social workers, registered dietitians, therapists and home health aides provide scheduled visits to ensure all of the patient’s needs are addressed. In addition, other compassionate care providers include:
- Volunteers who provide support, companionship and respite care services for patients, caregivers and loved ones.
- Spiritual Care Counselors who can assist with spiritual care and support.
- Bereavement Counselors who can provide assistance and support to caregivers and loved ones following the patient’s death.
Reverence brings the care to you or your loved one, wherever you are. Care options include continuous care, inpatient respite care and general inpatient care when there is an acute medical crisis and approval by the physician and the hospice team.
